Division:   Reach-Up (Welfare to Work) and WCMHS The Center for Counseling and Psychological Services (CCPS)

Program: Reach Up/WCMHS Outpatient Partnership

Reporting to: Director CCPS

Summary of the Role:

The Reach-Up Wellness Coach is a full-time 40 hour a week benefited position responsible for working with participants in collaboration with the Washington County Reach-Up Team as part of a welfare to work program that helps hard to employ welfare recipients move into lasting unsubsidized jobs. The Reach Up-Wellness Coach is under the general direction of the Washington county Reach-Up Supervisor, and the agency/clinical supervision of the Adult Outpatient Program at WCMHS. This position provides service coordination, referral, and consultation services to adults referred by the Washington County Reach Up Team, in need of mental health and/or substance abuse counseling.  The Reach Up Wellness Coach utilizes best practices based on assessed needs, goals, and client preferences. Wellness Coach services can include both individual and group supports.  Consistent individual supervision weekly, in addition to a weekly group supervision structure.

Responsibilities:

 

  • Provide integrated case management and service coordination.

  • Assist to develop, place, monitor and evaluate participants progress in substance abuse and mental health services as it relates to the Reach Up participants employment goals in conjunction with Reach Up employment services.

  • Coordinate with other services providers including family services, children's integrated services, employment services, schools and other clinicians.

  • Other duties as assigned by management.

    Qualifications:

     

  • Bachelor's Degree in related field preferred, or equivalent in education and experience.

  • General knowledge of community resources, Medicaid, standard models of service coordination, 12-step methodology, Co-Occurring Substance Abuse and Mental Health issues

  • Experience working in a variety of human service settings.

  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.

  • Successful completion of a criminal history background check .

    Physical Requirements:

     

    • Ability to drive to different client homes in the community using own vehicle.
  • Ability to stand/sit in a community and office-based setting for several hours.

  • Ability to life and move up to 20 pounds.

  • Ability to walk up to one mile in community-based setting.
