University of Virginia Registered Nurse (RN)- 23-hr Observation Extended Stay-Nights in Charlottesville, Virginia

Phase 3: Extended Stay (Post Anesthesia Care Unit / PACU / Fast Track / ESP)

Beds: 8 beds are staffed daily—staffing may be impacted on a day-to-day basis by staff call outs and leave. ESP follows the staffing ratios recommended by the American Society of PeriAnesthesia Nurses for all phases of PeriAnesthesia care. Nursing roles in Phase 3 focus on providing ongoing care for those transferred from Phase I and Phase II levels of care in PACU. General standards of care suggest a recommended Extended Stay nurse: Patient ratio of 1:3-5 based upon acuity. Current staffing ratios in Extended Stay Periop are 1:3-4.

Hours of Operation: Monday 0700 to Saturday 1900

Services: ESP is a ‘fast track’ observation unit for those low-acuity perianesthesia/perioperative patients requiring lower nurse to patient ratios to facilitate rapid patient turnover and ensure efficient patient progression through the perioperative department. Most patients stay less than 23 hours.

Cases for ESP are identified 24-72 hours in advance by Main Post-Anesthesia Care Unit (PACU) based on case posting information that includes postoperative destination, surgery type, and surgeon preference(s). The PACU

Charge nurse will also evaluate and identify possible ESP admissions in PACU during the recovery phase of care.

Volumes vary based on surgical volumes and case mix (i.e., surgical procedure types, patient acuity, etc.).

Admission/Discharge Criteria:

Admission: Post-anesthesia care is provided within this setting to those patients slated for same-day discharge or an observation period post-procedurally, as defined by the primary team (i.e, surgery) via order and/or note in EMR.

Phase 3 PACU outpatients from the Main OR and offsite locations require an extended period of post-anesthesia or post-procedural recovery (i.e., 4-23 hours) after Phase 1 recovery in PACU or Phase 2 care within PeriAnesthesia Main. Patients are to be cleared from Phase 1 anesthesia care per protocol/guideline or note from anesthesia provider.

Patient Populations: Outpatient cases 23-hour observation cases without significant comorbidities. Inpatient admissions requiring a 1-night stay without significant comorbidities or needing case management or extensive therapy needs

Capable clinician, focused on expanding knowledge and skills. Consistently provides effective direct care, as part of the interdisciplinary team, to a variety of complex patients. Manages care and implements treatment plans at a refined skill level in collaboration with patients, their families, physicians and other members of the health care team. Seeks as well as provides feedback for improved clinical practice. Assumes a beginning leadership role but seeks mentoring in this process.

Position Compensation Range: $39.90 - $53.00 Hourly

MINIMUM REQUIREMENTS

Education: Graduate of accredited nursing program, Bachelor of Science in Nursing required within 5 years of hire.

Experience: 1 year of relevant experience.

Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification required.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
