University Hospitals Health System Clinical Pharmacy Specialist, Psychiatric Medicine in Cleveland, Ohio

Description This position will be responsible for collaborating with various psychiatry teams at UH Cleveland Medical Center (UH CMC), including the Consult and Liaison Psychiatry Team, the Medical Psychiatry Team, and the Emergency Psychiatric Access Team. These teams provide care on a consult basis for patients in all areas at UH CMC including intensive care units, medical surgical units, MacDonald Women's Hospital, and the emergency department. This clinical pharmacy specialist will be expected to take an active role is medication management for these patient populations and will serve as a drug information resource for the teams. These teams are comprised of attending psychiatrists, resident psychiatrists, nurse practitioners, and medical students. The clinical pharmacy specialist will also be expected to serve as a preceptor for pharmacy residents and students in addition to serving as a member of the Residency Advisory Committee. This position will include additional local and system-wide committee involvement pertaining to the area of pharmacy and/or psychiatry. Duties will include, but are not limited to: Collaborating with various UH CMC psychiatry teams including the Consult and Liaison Psychiatry team, the Medical Psychiatry Team, and the Emergency Psychiatric Access Team Attending multidisciplinary team meetings daily Participating in local and system-wide medication-related committees Precepting pharmacy residents and students Providing didactic instruction to medical students and psychiatry residents Taking an active role in formulary management to ensure the safe and effective use of psychotropic medication Serving as a clinical resource for pharmacy and other healthcare professionals in the area Psychiatric Medicine Essential Functions Provide Clinical Pharmacy Services (drug information, drug therapy monitoring, patient specific consultations, healthcare staff in-services pharmacoepidemiology and pharmacoeconomic evaluations in selected patient populations) Investigate/research/report adverse drug reactions Participate in Medication Safety and Therapeutics (MS&T)Committee process: evaluate requests for new drug usage and old formulary drug relevance and review therapeutic classes for relevance, assist in updating hospital formulary Serve as an informational and patient problem solving resource for clinical staff pharmacists. Provide continuing education to clinical staff pharmacists and precept learners Required For All Jobs Performs other duties as assigned. Complies with all policies and standards.
