71120BRAuto req ID:71120BRDepartment:34718 - DK Physical Therapy Job Description:Evaluates and treats patients/residents upon physician referral, incorporating indications and contraindications, physician's request, diagnosis, test and measurement results, and patient's medical history into the plan of treatment;Forms treatment protocols designed to achieve goals in a specific measurable period of time. Treats patients and residents with the goal of returning them to at least their prior level of function. Discharges patients appropriately when the patient has reached a plateau;Daily notes, AHT documentation with corresponding rehab paperwork, and charge doccumentation should be completed as soon as possible on the day the patient receives treatment. Initial evaluations must be completed within 24 hours of the assessment;Documentation forms must be complete. If specific information could not be obtained, the reason should be noted. The therapist should utilize only standard abbreviations in documentation;Communicates, either verbally or by written means, with patient/resident, their family, physician and other members of medical staff. Functions as part of a continuum of health care providers to meet the needs of our patients;Provides LTC residents initial, quarterly and annual evaluations, also screens those residents to determine the appropriateness for restorative therapy;Complies with insurance company regulations to obtain approval for continued patient treatments and reimbursement.Attends meetings, attend and provide in-services, and participate on committees as requested;Notifies supervisor if equipment is not working. Maintains a clean workspace by sanitizing equipment and treatment area, changing linen and putting away equipment when treatment is complete or delegates said responsibilities and oversees the completion of these tasks;As required by their professional organization, attends seminars to enhance professional skills;Supervises physical therapist assistants and ROM aides;Able to perform basic scheduling and other clerical work in the absence of the rehabilitation secretaries;After one year of experience, participates in student intern program as directed by the Coordinator of Clinical Education for Physical Therapy;Follows the Caring First Standards of Performance, follows the chain of command and positively resolves conflict with others politely and privately;As the rehabilitation department operates in several different areas on two different campuses, therapists must exhibit a willingness to work in departments other than their primary department, should the need arise.Location:Brooks Memorial Hospital Recruiter:Vanessa BorowiakTitle:Physical TherapistMinimum or Preferred Qualifications:Bachelor of Science from approved School of Physical Therapy by the APTA or higher academic degree (Masters) or minimal level of education changes. If a Doctorate of Physical Therapy becomes the minimal education requirement, to practice Physical Therapy in NYS or the USA, then that level is the TLC educational minimal requirement along with appropriate registration and certification (this would apply to new graduates). : If new graduate, eligible for New York State licensure exam, must have applied for temporary license. Must be compassionate, patient, and understanding.
