Upper Great Lakes Family Health Center Physician in Hancock, Michigan

Upper Great Lakes Family Health Center in Hancock, MI seeks Physician for Internal medicine/geriatric physician on all adults 18 and over. Responsible for delivery of quality medical care to patients of the Center and functions as a member of the multi-disciplinary health care team within the scope of his or her medical practice. Provide clinical services and clinical oversight under the supervision of the Medical Director. Maintain clinical expertise and competency (to include age specific competency) as well as demonstrates thorough knowledge of procedures to deliver quality primary health care. Provide clinic sessions, as scheduled, in direct patient care at clinic site, including extended hours clinics and other outreach locations as direct and in accordance with assigned full-time equivalence (FTE). Employ non-surgical methods and procedures to diagnose and treat patients' medical conditions. Consult with patients to understand their symptoms and health concerns and diagnose and treat acute illnesses including bacterial or viral infections and chronic diseases and conditions including hypertension, heart disease, or diabetes. Prescribe or administer medication, therapy, and other specialized medical care to treat or prevent illness, disease, or injury. Administer immunizations, explain procedures, and discuss test results or prescribed treatments with patients. Internal medicine physicians maintain detailed patient notes and write patient referrals when necessary. Research the testing and development of new medications, treatment methods, or procedures to prevent or control illness, disease, or injury. Geriatric doctors, also called geriatricians, specialize in caring for aging adults who often have complex medical issues. Focus particularly on keeping patient functional and help maintain their quality of life. Examine, diagnose, treat and/or refer patients assigned to physician's panel, on an in-patient and out-patient basis (as per individual employment agreements); prescribe pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions. Refer clients for appropriate specialty care services, lab and x-ray and any other ancillary services that are appropriate for patient's management and care. Review all lab and x-ray reports for patients under his/her care on a timely basis and makes medical management decisions appropriately. Follows appropriate standards of care for each patient. Provide continuity of care to center patients on his or her patient panel and other patients as assigned, including routine in-patient, outreach services, home-visits and nursing home / foster home care to center patients, and school-based clinics to include injury assessment and treatment and injury prevention and education, as needed. Perform emergency on-call on a scheduled basis with other physicians within the group and if required by the local hospital(s). Participates in after-hours phone coverage for UGLFHC patients if needed. Maintain a problem-oriented health record for each patient seen in accordance with the Center's established Health Information Management policies and procedures. Ensure that patient and family/significant others are educated about diagnosis and the plan of care. Act as a resource for facilitating communication when necessary. Participate in specified health promotion, education and/or prevention programs as needed (i.e. diabetes collaborative/health fairs, school-based injury prevention and concussion education, etc.); attend and participates in clinic meetings / departmental meetings and other clinic functions as required by the Center. Assumes responsibility/accountability for delegating, directing, and supervising patient care activities. Geriatric doctors understand caregivers' roles and work with family members, too.
