Description:

Position Purpose

At Saint Francis Hospital the Case Management Department helps patients coordinate and navigate through their Health care needs while determining how to provide that care efficiently to control costs.

What you will do:

  • Provide patient Centered Leadership while ensuring effective utilization management, compliance with Medicare requirements, and the development of initiatives to reduce readmissions.

  • Maintain and monitor operating statistics for the Department adjusting staffing, supplies or capital expenditures.

  • Recommend the annual capital, salary and non-salary budget for the Department of Care Management.

Minimum Qualfications:

  • Education : Bachelors in Nursing - Master's Degree Preferred

  • Licensure : Current Licensure as a registered nurse (RN) in Connecticut

  • Experience : Five or more years of progressive experience in a hospital / health system setting in a leadership role in case management or a related role

  • Must be knowledgeable regarding state and federal health regulations, reporting requirements and health care reimbursement systems.

  • Basic knowledge and application of statistical analysis, DRG's/ICD9/10, Medical Necessity criteria, JCAHO and CMS requirements for discharge planning and utilization management
