Job Summary: The SNF LPN Care Coordinator will be an essential role on the new SNF care team. This nurse will work under direct supervision of the provider's care for residents. This dynamic position entails hands-on face-to-face care within local SNFs as well as virtual, and work from home job responsibilities. The SNF LPN Care Coordinator assists with care planning, coordination of care, patient education, and care team communication on behalf of nursing home residents. This hybrid position requires skill in physical examination skills, adept use of technology, great collaboration skills, willingness to travel to local SNF facilities, and to contribute as a leader on the SNF care team. We anticipate that this position will be 70% in SNF facility, and 30% work from home.

Aptitudes: Ability to learn patient history and physical examination required for SNF resident care. Ability to investigate and observe patients to help identify holistic patient care needs. Ability and willingness to acquire knowledge of care quality data element and metric definitions as they pertain to SNF care. Ability to work with complex and/or high-risk patients, including those with behavioral and cognitive issues. Ability to achieve cognitive, organization and emotional maturity to deal effectively with multiple tasks, stresses, deadlines, difficult situations and/or customers.
