Highlights

Community Health Network is seeking a Psychiatric-Mental Health Nurse Practitioner (PMHNP) to join the Community Behavioral Health team in the Community Mental Health Center setting. This provider will practice within Community’s outpatient clinic located in Kokomo, Indiana, and will work with both the adult and youth populations. This position also entails providing medication-assisted treatment (MAT). Ideal candidates will be highly accountable, independent, and have a desire to deliver direct, collaborative patient care.

Licensure & Certification

Educational background consistent with requirements for licensure and certification within the state of Indiana.
