POSITION SUMMARY

This crucial role is the clinical and P&L leader for the foundational business unit of our care model – the center. The Senior Medical Director (SMD) will partner with the Center Director/Center Executive Director to form the dyad that is fully accountable for center clinical, operational, and financial performance. The SMD is an earned title where the Center Medical Director completes all necessary primary care physician (PCP) and medical director training, plus demonstrates 1) success in developing providers to deliver high quality, cost-effective care; 2) effective dyad partnership with the Center Director/Center Executive director; and 3) ability to drive center performance. The SMD role will report directly to the Regional Chief Medical Officer.

We are an outcomes-focused, value-based organization. This position will be subject to, and have center oversight for, the following metrics/competencies that are regularly assessed to help our clinicians become successful and reach PCP Partnership status: panel size target of 400-450 patients for each full time provider, patient outcomes (admissions and hospital sick days), CG CAHPS and net promoter score (patient experience), HEDIS and clinical gaps closures, and medical cost effectiveness measures. Each PCP will have goals for these metrics as part of the center goals. Culture is also very important at ChenMed - SMD are accountable to ensure PCPs are leaders in our organization and champion a positive culture of love, accountability, and passion.

As a player/coach, the SMD will demonstrate and lead the center’s providers towards:

 Accountability and oversight for outcomes: The SMD demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of at least 200-225 risk adjusted patients (50% partial patient panel, 50% admin time). They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviors.

 Coaching for health: Act as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partner with the patient to work toward the goals, and frequently follow up on those goals on the path to improved health for their patients.

 Simplifying for action: Simplify and prioritize appropriately so that behavior change is more actionable and attainable, both for the patient in helping them achieve their goals, and when leading care team towards their performance goals.

The SMD will be the center clinical leader:

 Manage day-to-day clinical and business operations to improve the clinical and financial performance of the center, as the true dyad partner to the Center Manager/Center General Manager.

 Directly supervise, performance manage and train PCPs in his/her assigned center towards being accountable for maximizing overall clinical value by improving clinical quality, efficiency, outcomes, cost effectiveness, and physician/patient satisfaction. This will be accomplished by mentoring and coaching each provider as part of a defined development plan, which includes monthly leadership rounding.

 Lead development of PCPs towards partnership attainment and/or leadership roles.

 Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly care meetings.

 Drive a positive culture that supports a cohesive team approach, drives excellent results and relationships, and promotes "best place to work" culture measured by employee engagement scores.

 Build effective relationships with community and lead community engagement in a way that promotes strong membership growth and retention for the center.

SMD will have demonstrated mastery of:

• PCP Black Belt Training and PCP Essentials

• Medical Director Essentials and Center Operations

• Medical Economics and Center Financial Performance

• Driving Membership Growth

The SMD will be required to function both independently and in collaboration with other health care professionals. He/she will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. The SMD will work closely with the applicable center and market leadership to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health as well as market/company goals.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES FOR OWN PANEL AND CENTER:

  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination and documentation for patients with acute and complex chronic health needs.

  • Manages clinical teams to deliver VIP service, standards of care and quality guidelines, and HEDIS/STARS

  • Inspire seamless coverage of clinical staff for each other (e.g., PTO, overflow) – patients will belong with both the PCP as well as the center.

  • Leads SH and Transforming Care Meeting (TCM) weekly, as well as supports Center Manager/Center General Manager in other center meetings. SMD partners with Center Manager/Center General Manager to co-lead center stand-ups.

  • Managing, mentoring and coaching PCPs in his/her assigned center to deliver outstandingclinical outcomes, including sampling other PCP’s daily huddles within their center.

  • Appropriate management of hospital sick days for center as follows: reviews inpatient census daily, delivers feedback to PCPs and ensures PCPs are keeping in contact with hospitalized patients and hospitalists; facilitates review of inpatients and other high priority patients during SH, accountable for discharged patients being seen and coaches PCP on appropriate flagging of high priority patients. Engages with the hospitalist whenever patients are in the hospital (regardless of whether the hospitalist works for ChenMed or not).

  • For patients that are unable to come to the office—in hospital, SNF, LTC or homebound, the SMD will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources (e.g., telehealth) that may be available depending on the region.

  • Collaborates with Regional Chief Clinical Officer (RCCO) in training of new primary care practitioners.

  • Reviews/approves center-level referrals and provides back-up for regional referral and delegated utilization authorizations. Appropriately seek consultation from specialists when needed; remains involved in, and responsible for, the detailed care of the patient.

  • Takes an active role as needed in recruiting patients for the center and additional providers for the company.

  • Other duties as assigned and modified at RCMO’s discretion, which may include:

  • Assists RCMO with region quality, medical cost efficiency, and performance improvement initiatives.

  • Oversees monthly scorecard reviews and in conjunction with the MCMO delivery of quarterly CSE analysis.

  • Uses the understanding of the local region dynamics to drive clinical initiatives.

  • Develops deep relationships with providers and key stakeholders in the region.

  • Builds clinical credibility and trust to deepen external relationships. Assist and coordinate, in conjunction with the transitional care team; hospital, SNF, post-acute, UM, and home-based services for the division.

  • Assists RCCO with interviewing PCP and specialist candidates (Note: RCCO makes hiring decisions).

  • Provides training in other ChenMed entities, as needed.

  • Other key relationships: Other cross-functional regional leaders, external sub-specialists and providers in hospitals and post-acute care facilities.

  • Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company

  • Ability and willingness to travel locally as needed in their region, if applicable, then occasionally (up to 20%) regionally and nationally for recruiting or training purposes

  • Fluency in English, verbal and written. There may be jobs in some centers that require fluency in other languages, and this will be made known at the time of application

  • This job requires use and exercise of independent judgment

EDUCATION AND EXPERIENCE CRITERIA:

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required

  • Current, active MD licensure in state of employment is required

  • Board Eligibility is required; Board Certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred

  • Once Board certified, SMD will maintain board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required

  • Must have a current DEA number for schedule II-V controlled substances

  • A minimum of 2 years’ clinical experience in geriatric, adult or family practice setting preferred

  • A minimum of 3 years’ clinical leadership experience is preferred

  • Must have completed all internal physician training and have attained partnership.

  • Must complete full medical director training prior to promotion to Senior Medical Director, as part of being a successful Center Medical Director for at least 1-2 years

  • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment ​
