Summary: Under the direction and supervision of the Director of Patient Services and in accordance with the physician’s orders the RN Case Manager is responsible for planning, organizing, implementing and evaluating the plan of care. The RN Case Manager provides direct patient care and education, and is responsible for coordination of services and ensuring communication with the physician and among the multidisciplinary care team.

Essential Functions

Overall responsibilities include providing and coordinating quality patient care, maintaining patient caseload, maintaining productivity goals with and accurate, timely documentation. The RN Case Manager is responsible for the overall plan of care and the outcome of that care, including care delivered by other nursing staff (LPN, HHA) who must be directed and supervised by the RN.

Minimum Qualifications:

• Associates degree in Nursing; BSN preferred and unencumbered NYS RN license

• Minimum one year home care experience is preferred

• Knowledge of State and Federal regulations, as well as payer guidelines.

• NYS Driver’s License

• CPR Certification

• Demonstrate critical thinking skills

• Strong communication, organizational and time management skills.

• Strong electronic (EMR) documentation skills.
