Description

Under general supervision, provides non-medical, in-home support services to program participants and support to office staff and case managers. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.

Special Requirements

  • Must have own vehicle to drive to client’s homes. May be multiple in one day.

  • Must be able to lift 20 pounds. Must be able to climb stairs.

Essential Duties and Responsibilities

  • Provides housekeeping or shopping services to clients.

  • Make and change beds; dusting, vacuuming, light cleaning of kitchen, bedroom, bathroom; dishwashing; prepare light meals; make list of needed cleaning items or grocery items; laundering, ironing; grocery shopping

  • Provides supervision services to clients (respite for caregivers).

  • Monitor client’s safety.

  • Maintains accurate activity records.

  • Follows established procedures and policies for programs.

  • Completes case notes electronically.

  • Keeps accurate mileage and timesheet records.

  • Utilizes web-based client database for client information and recording of units of service.

  • Maintains up to date client service schedule on agency system (currently Outlook).

  • Tracks any client payments received.

  • Supports department operations

  • Back up for phones, report preparation, mail, on call coverage, and data maintenance.

  • Administrative tasks such as copying, faxing, organizing, and chart maintenance.

  • Assist with department meetings and events

  • Participates in relevant Agency meetings and/or trainings

  • Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.

  • Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse

  • Participates in relevant Agency meetings and/or trainings

  • Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.

Qualifications

Education: Associate degree in Human Services or related field preferred. Equivalent combination of education and experience will be considered

Credentials: Valid and clean NYS Driver’s License.

Experience: One year of experience in human services required. Experience in geriatrics preferred. Bilingual (Spanish, Burmese, Karen, Nepali, Somali or Arabic) desirable.

Compliance: Adheres to all applicable federal and state laws and regulations including, but not limited to, those governing confidentiality, privacy, program, billing and documentation standards. All duties must be performed in accordance with CCFCS’s corporate compliance & ethics program.

Medical Requirements:

  • Proof of immunization to rubella, consistent with good medical practice, except that woman of childbearing age shall have a screening test approve by the NYS Dept. of Health, to be followed by immunization as appropriate.

  • PPD (Mantoux) skins test for TB prior to employment and no less than every 2 years thereafter, for negative results. Positive findings will require appropriate clinical follow up.

  • An annual or more frequent, if necessary, health status assessment to assure that all personnel are free of any health impairment that is on potential risk to the client, family, or may interfere with the performance of job duties.
