Our | | | community health based model | | | allows our therapists to work in | | | the PACE outpatient center and in | | | participants' homes as needed. | | | This role also implements plan of | | | care with input from PT as | | | needed, regularly informs the | | | interdisciplinary team (IDT) of | | | the medical, functional, and | | | psychosocial condition of | | | participants, remains alert to | | | pertinent input from other team | | | members and participant | | | caregivers, and documents changes | | | in a participant's medical record | | | consistent with documentation | | | policies. | | +-----------------------------------+-----------------------------------+ | Position Requirements | | +-----------------------------------+-----------------------------------+ | *Education - *Graduate of | | | physical therapy assistant | | | program with curriculum approved | | | by the American Physical Therapy | | | Association. | | | *Experience - *At least one | | | year experience working with the | | | elderly preferred. | | | *Other Requirement(s) | | | - *Licensed as physical therapy | | | assistant in Washington | | | State. Must obtain and maintain | | | CPR certification. | | +-----------------------------------+-----------------------------------+
