Mental Health Partners Community-Based Family Health & Wellness Coach | Bilingual Spanish in United States

Purpose:

As our Family Health and Wellness Coach you will work with children, youth, and caregivers with high mental health and co-occurring substance use difficulties to teach and coach skills and strategies that support the family unit in fostering and maintaining healthy habits, healthy behaviors, healthy relationships, and healthy minds.

Job Summary:

Empower youth and families with short-term support services, including psychoeducation, psychosocial rehabilitation, and skill coaching. Tailored interventions will address emotional regulation, crisis and behavior management, effective parenting strategies, relationship skills and other relevant areas. Support will primarily take place in the home and community, fostering real-world application and enhancing everyday functioning.

Job Profile:

+ + Provide services using an approach that is warm, non-threatening, and non-judgmental.

  • Foster connection to help youth and families to engage in the process.

  • Teach and coach parenting skills with a challenging population.

  • Provide education about mental health and substance use disorders and symptoms that are impacting a family.

  • Collaborate and coordinate with child welfare services, probation, schools, juvenile justice, hospitals, and other external partners to meet the needs of the client & family.

  • Participate in consultation and supervision with team members and other professionals in the organization.

  • Maintain thorough, accurate, and timely documentation per clinical and administrative policies and procedures.

  • Promote, support, and demonstrate MHP’s mission, vision, and values through behavior, job performance and adherence to policies, procedures, and standards.

  • Participate in the development and improvement of programs and business processes.

  • Work some evening hours to meet families and youth when they are available.

What We Need:

+ + BA degree in Social Work, Counseling, Psychology, or a related health care field, from an accredited institution.

  • If the BA degree is not in one of the preferred fields above, at least one (1) year experience in a behavioral health-related field is required.

  • Entry level mental health and/or substance abuse treatment experience with children, teens and parents.

  • Ability to meet direct service expectations and to complete documentation within required timeframes.

  • Ability to develop knowledge of community resources and make appropriate referrals.

  • Ability to manage difficult or emotional situations.

  • Must be 21 years old, have a safe driving record, a current Colorado driver’s license and ability to travel within Boulder and Broomfield Counties.

  • Health and wellness evidence-based practices knowledge and experience is preferred.

  • Previous experience leading groups strongly preferred.

  • Must be Bicultural/Bilingual (Spanish)
