Mental Health Partners Health and Wellness Coach in United States

Purpose:

The Health and Wellness Coach models a healthy lifestyle, motivates, and educates clients with significant mental health conditions and their caregivers to engage in their treatment options as well as in sustainable behavior change so that they can achieve their whole health goals.

Job Summary:

The Health and Wellness Coach will guide families of youth with significant mental health conditions and concerning co-occurring health conditions in behavior activation strategies to increase safety and protective factors for youth/families and their environment for goal of family preservation or for increased independence for transition age youth. This work requires skills working with parents and youth who may be difficult to engage in behavior change and who are challenged with socio-emotional, attachment, and other related behavioral, substance use and physical health issues. This position will use behavior modification support strategies, parent or youth psychoeducation and skills coaching, and family interventions that are proactive and focused on improving functioning of individual and family.

Essential Functions:

Roughly 70% of time is spent on Health and Wellness Coach duties, 20% on Care Coordination duties, and10% on Client Service Advising duties.

  • Conducts classes/groups in partnership with other health professionals aimed at increasing parent or youth behavioral, physical, or social functioning and wellness.

  • Helps youth and families develop strategies to identify and meet individual and family goals

  • Helps clients develop and practice life-skills to monitor and manage symptoms and improve daily functioning and success at home, school, and in the community.

  • Connects clients with peer supports, community resources, and crisis intervention.

  • Promotes importance of social supports and good lifestyle choices.

  • Partners with community agencies to connect clients to relevant community resources, with the goal of enhancing client health and well-being, increasing client satisfaction, and minimizing the delivery of redundant services that are already available elsewhere.

  • Educates clients, families/caregivers, and other staff about relevant community resources.

  • Coaches on engagement in treatment and/or increase client’s ownership of their recovery and to help them focus on health vs. illness.

  • Conducts supportive counseling with evidence based brief interventions to help with proactive measures to avoid crisis, de-escalation techniques within a crisis and to assist with social, interpersonal and skill development.

  • Arranges for assessment/re-assessment for additional services or care if a client’s needs fall beyond scope of practice.

  • Contributes goals and objectives to care plan when client is involved in specialty care and develops care plan in full when Health and Wellness Coaching is the only service engaged in beyond care coordination.

  • May assist with the collection and tracking of client’s vitals and other outcomes as needed

  • Accompanies clients to medication evaluations as needed and may transport clients to appointments as needed.

  • Maintains thorough, accurate and timely documentation of the provision of services, assessments, evaluations, and progress toward the completion of client’s treatment goals consistent with clinical and administrative policies and procedures

Education/Experience/License/Certification

  • BA degree required preferably in social work, counseling, psychology, or a related health care field, from an accredited institution

  • Entry level mental health and/or substance abuse treatment and diagnostic skills strongly preferred

  • Previous experience leading groups strongly preferred

  • Prior experience in a physical health or educational setting and training is desired

  • Health and wellness evidence-based practices knowledge and experience is preferred

  • Prior experience with education curriculum development and implementation preferred

  • Must be 21 years old, have safe driving record, a current Colorado driver’s license and ability to travel within Boulder and Broomfield Counties
