ABOUT THE POSITION

At Goodwin Hospice, we're seeking a Bereavement Counselor who provides bereavement services to the bereaved including family and community members. The Bereavement Counselor provides support based on plan of care that may include phone calls, mailings and supportive counseling services both virtually and in person for individuals and groups.

DUTIES

  1. Assists in implementing the hospice Bereavement program in accordance with federal regulations and Goodwin Hospice policies to provide support to the bereaved, facility staff and hospice team members, when needed.

    1. Coordinates care with the hospice interdisciplinary team and participates in team meetings.

    2. Develops and coordinates bereavement plan of care for a caseload of primary bereaved including caregiver’s, friends, and family members.

    3. Coordinates all aspects of the bereavement needs for assigned caseload including sending condolence cards and mailings, bereavement counseling via phone, virtual platform or face to face based on psychosocial and bereavement assessments, facilitating groups/workshops, and linkages to community resources.

    4. Documents the type of bereavement follow-up offered and provided through progress notes in EMR for up to thirteen months after the patient death.

    5. Collaborates with bereavement counseling colleagues to provide effective education materials, support groups, workshops and community outreach.

    6. Educates hospice volunteers regarding roles and responsibilities of BR counselor and collaborates with volunteer coordinator as needed.

    7. Is an active participant in the development, planning, and presentation of the Remembrance Day Programs.

    8. Ability to travel throughout Northern Virginia area.

    9. Performs additional job duties as assigned and directed.

QUALIFICATIONS

  1. Must possess a Master’s Degree from an accredited university in Social Work or counseling-related field. License eligible or licensed in the Commonwealth of Virginia.

    1. Three (3) years of relevant experience in Social Work preferred, with at least one (1) year working with hospice.

    2. Must possess a Master’s Degree from an accredited university in Social Work or counseling-related field. License eligible or licensed in the Commonwealth of Virginia.

    3. Three (3) years of relevant experience in Social Work and/or Bereavement Counseling, with at least one (1) year working with a hospice. Knowledge regarding grief and loss, experience in working with bereaved individuals and grief reactions is critical.

    5. Must possess ability to deal tactfully and effectively with residents, staff, family members and the public.

    6. Must possess excellent technology skills to master electronic health records, different applications including Word, Excel, PowerPoint, Zoom and Microsoft Teams.

    7. Ability to work within an interdisciplinary team is required.
