Providence St. Joseph Health Clinical Pharmacist - FAM MED in Walla Walla, Washington

Description The Clinical Pharmacy Specialist (CPS) in the Ambulatory Care setting will provide care to patients of the Providence Medical Group with emphasis on Population Health initiatives. The CPS role is designed to be part of the Medical Home in primary care which is: patient-centered, comprehensive, team-based, coordinated, accessible, and focused on quality and safety. The CPS will provide coordinated, comprehensive, high-quality care to patients. The responsibilities of the clinical pharmacy specialist will be divided into: (1) Direct patient care, (2) pharmacy clinical support and assistance with hospital discharge facilitation for high-risk patients, (3) program and policy development and leadership, (4) preceptorship/education, (5) Population Health and Medical Home projects. Completion of ASHP accredited pharmacy practice residency - AND - Ambulatory care Pharmacy Specialty residency OR Completion of ASHP accredited pharmacy practice residency -OR- ambulatory care pharmacy specialty residency -AND- (1) year work experience in either hospital or ambulatory care pharmacy practice OR Completion of ASHP accredited pharmacy practice residency with ambulatory care focus OR Experience equivalent to above. Preferred Qualifications: 2 years of Clinical pharmacy practice work experience with provision of staff education and development, leadership, and program design Why Join Providence?
